  9. This is a request under the Freedom of Information Act.

  11.     I request a complete and thorough search of all filing 
  12. systems and locations for all records maintained by your agency 
  13. pertaining to and/or captioned:

  33.     This request specifically includes "main" files and "see 
  34. references," including, but not limited to numbered and lettered 
  35. sub files, "DO NOT FILE" files, and control files. I also request 
  36. a search of the ELSUR Index,a nd the COINTELPRO Index. I request 
  37. that all records be produced with the administrative pges.

  39.     I wish to be sent copies of "see reference" cards, 
  40. abstracts, search slips, including search slips used to process 
  41. this request, file covers, multiple copies of the same documents 
  42. if they appear in a file, and tapes of any electronic 
  43. surveillances.
  44.  
  45.     I wish to make it clear that I want all records in you 
  46. roffice "identifiable with my request," even though reports on 
  47. those records have been sent to Headquarters and even though 
  48. there may be duplication between the two sets of fils. I do not 
  49. want just "interim" documents. I want all documents as they 
  50. appear in the "main" files and "see references" of all units of 
  51. your agency.

  53.     If documents are denied in whole or in part, please 
  54. specify which exemption(s) is(are) claimed for each passage or 
  55. whole document denied. Please provide a complete itemized 
  56. inventory and a detailed factual justification of total or 
  57. partial denial of documents. Give the number of pages in each 
  58. document and the total number of pages pertaining to this 
  59. request. For "classified" material denied pleae include the 
  60. following information: the classification (confidential, secret 
  61. or top secret); identity of the classifer; date or event for 
  62. automatic declassification, classification review, or down-
  63. grading; if applicable, identity of official authorizing 
  64. extension of automatic declassification or review; and if 
  65. applicable, the reason for extended classification.

  67.     I request that excized material be "blacked out" rather 
  68. thatn "whited out" or cut out and that the remaining non-exempt 
  69. portions of documents will be released as provided under the 
  70. Freedom of Information Act.

  72.     Please send a memo (copy to me) to the appropriate units 
  73. in your office to assure that no records related to this request 
  74. are destroyed. Please advise of any destruction of records and 
  75. include the date of and authority for such destruction.

  77.       As I expect to appeal any denials, please specify the 
  78. office and addressto which an appeal should be directed.
  79.  
  80.       I believe my request qualifies for a waiver of fees since 
  81. the release of the requested information would primarily benefit 
  82. the general public and be "in the public interest."
